Individuals

WiseSimplicity
Coverage for the big stuff. This plan may be for you if it’s important for your monthly rate to remain low and you’re willing to pay more out-of-pocket. Plan provides up-front coverage for one preventive care exam with deductible waived per year.

WiseEssentials
Great basic coverage. A good option when you’re looking for a low rate and more up-front coverage for routine care needs. This plan provides up-front coverage for your first six office visits.

WiseEssentials Copay
The ease and predictability of a copay. Similar to WiseEssentials, this plan offers higher annual deductibles, which lowers your monthly rate. Plan covers your first three office visits with $25 copay.

WiseSavings 
A Health Savings Account (HSA) plan. Quality healthcare coverage and the opportunity to save money on a pre-tax basis for future qualified medical expenses. Make your money work for you with a WiseSavings HSA.

WiseChoices
Offers the widest range of covered benefits. A benefit-rich plan that includes coverage for pharmacy, vision care and maternity. A nice option if you’re looking to cover all your bases.

Employers

LifeWise’s flexible plan designs allow us to work closely with employers to provide a solution that meets the unique needs of your group. The following products are currently available:

• Group Life Insurance - provides benefits to a beneficiary in the event of an employee’s death from any cause
• Accidental Death & Dismemberment (AD&D) - provides benefits in the event of accidental death or dismemberment
• Dependent Life - provides benefits to the employee in the event of dependent’s death(s)
• Short-Term (STD) and Long-Term Disability (LTD) Income Insurance - provides benefits for income replacement in the event an employee is unable to work due to an illness or injury
• Voluntary Products - additional payroll deduction benefit options for employees, including voluntary Life Insurance, Accidental Death and Dismemberment, Long Term Disability and Short Term Disability
• Medical Stop Loss - employer reinsurance for self-funded medical plans

Medicare Plans

LifeWise offers a range of plan options that meet a variety of budgets and needs. With LifeWise Medicare Supplement coverage, you have the freedom to choose your doctors and hospitals. You can receive care anywhere in the United States—and most plans include worldwide emergency coverage.

Choose From Four Plans:

1. Plan A provides coverage for basic benefits.
2. Plan C adds coverage for skilled nursing, deductibles and foreign travel emergency.
3. Plan F includes all that Plans A and C cover plus 100 percent of Part B Excess.
4. Plan I includes all that A, C and F cover, except for the Part B deductible. Plus, Plan I adds coverage for at-home recovery.
