Horizon Individual Direct Access Plan A/50 70/50
Their new Horizon Individual Direct Access Plan A/50 70/50 offers access to network providers, and gives you additional freedom to seek care outside the network with no referrals.

Horizon Individual Direct Access Plan C 80/70
Our new Horizon Individual Direct Access Plan C 80/70 gives you a higher percentage of coverage versus Horizon Individual Direct Access Plan A/50 70/50; all at a competitive monthly rate. Under this plan, you also have access to network providers, and additional freedom to seek care outside the network with no referrals.

Horizon Direct Access Plan C 100/70
Our new Horizon Individual Direct Access Plan C 100/70 provides you with the highest percentage of coverage versus the other Direct Access plans. This plan has no in-network deductible, copayments for certain services and 100% coverage for other types of services

Horizon HMO*This Managed Care plan can give you the security of low out-of-pocket expenses, comprehensive coverage, and virtually no claim forms to fill out; all at a competitive monthly rate. All your medical care is managed through your Primary Care Physician (PCP), whom you choose from our network of more than 10,000 doctors in the state.

Horizon Basic and Essential EPO and EPO Plus
Horizon Basic and Essential EPO and EPO Plus are Exclusive Provider Organization plan designs. In order to receive coverage, eligible members must use in-network providers participating in the Horizon Managed Care Network (except in the case of medical emergencies).

Horizon Basic Plan A/50
Horizon Basic Plan A/50 gives you the freedom to select and use any doctor or hospital for your care. You choose an annual deductible of $1,000, $2,500, $5,000 or $10,000 with a 50% coinsurance.

Horizon Traditional Plan B
This is a traditional fee-for-service health benefits plan. Under Plan B, you can choose from annual deductibles of $1,000 and $2,500 with a 60% coinsurance. Use any physician you choose.

Horizon Traditional Plan C
Horizon Traditional Plan C is a traditional fee-for-service plan that gives you a higher percentage of coverage than our Traditional Plan B; all at a competitive monthly rate. Under Plan C, you can choose from annual deductibles of $1,000 and $2,500 with a 70% coinsurance and use any physician you choose.

Horizon Traditional Plan D
Horizon Traditional Plan D provides you with the highest percentage of coverage of all the traditional fee-for-service plans. Under Plan D, you can choose from annual deductibles of $1,000 and $2,500 with an 80% coinsurance and can use any physician you choose.

Horizon Advantage Plans
The Preferred Provider Organization (PPO) health plan offers the freedom for members to use any of the more than 17,000 participating physicians, hospitals and other health care professionals in the Horizon Traditional Network.

Horizon MyWaySM HAS
Horizon MyWay HSA is a consumer-driven health plan that combines a qualified high-deductible health plan and a Health Savings Account (HSA). The health plan can either be a Horizon PPO or Horizon Direct Access plan.

Horizon BCBSNJ Basic Health Plan A
This is a fee-for-service health benefits plan under which you and your employees can receive limited (30 inpatient days per year) hospital coverage at a lower cost than other, more comprehensive health benefits plans.

Horizon BCBSNJ Comprehensive Health Plan B
This comprehensive fee-for-service health benefits plan provides more comprehensive coverage than Basic Health Plan A.

Horizon BCBSNJ Comprehensive Health Plan C
This comprehensive fee-for-service health benefits plan covers you at a higher percentage than Plans A or B; all at a competitive monthly rate.

Horizon BCBSNJ Comprehensive Health Plan D
This comprehensive fee-for-service health benefits plan covers you at a higher percentage than Plans A, B, or C; all at a very competitive rate.

Horizon BCBSNJ Comprehensive Health Plan E
This comprehensive fee-for-service health benefits plan gives you a higher percentage of coverage than Plans A, B, C, or D.

Horizon BCBSNJ PPO
Horizon PPO is our Preferred Provider Organization plan. Your employees have the choice of more than 24,800 physicians and specialists in our Horizon PPO Network and 73 hospitals in more than 82 locations.

Horizon HMO Access
Horizon HMO Access provides direct access to network specialists without a referral.

Horizon BCBSNJ HMO
This Managed Care plan gives your employees the security of low out-of-pocket expenses, comprehensive coverage, and virtually no claim forms to fill out; all at a competitive monthly rate.

Horizon BCBSNJ POS
You and your employees have the freedom to use the Horizon Managed Care Network of physicians or choose physicians and hospitals outside the network.

Fraud Investigations Have Reduced Health Care Costs By $67 Million

(Newark, NJ, June 30, 2009) - Since 2006, Horizon Blue Cross Blue Shield of New Jersey’s (Horizon BCBSNJ) Special Investigation Unit (SIU) has recovered or saved $67 million for the company’s 3.6 million members.
In 2006, Horizon BCBSNJ’s SIU received the “Excellence in Investigating Fraud Award” from the New Jersey Attorney General’s Office for its diligence in preventing and uncovering health care fraud. (Continue Reading)
