Fidelis offers the following Health Options for Individuals and Families in the New York area:

Medicaid
Health coverage you can count on for you and your family -- at no cost. Fidelis Care Medicaid is an easy-to-use health plan that provides personal care from your own doctor, hospital and emergency care, maternity care (ask about our InfantCare and BabyCare programs), dental care (in some areas), eye exams, eye glasses, and more.

Child Health Plus
Quality, affordable care for New York's kids up to the age of 19. As part of New York State's Child Health Plus program, kids are covered for regular checkups, immunizations, routine dental and eye care, prescriptions, hospital and emergency care and more -- all the care they need to keep them happy and healthy.

Family Health Plus
Complete health coverage for adults, ages 19-64. Fidelis Care Family Health Plus is part of New York State's Family Health Plus program for uninsured adults who are residents of New York State and are not eligible for Medicaid. Members choose their own doctor and are covered for hospital and emergency care, prescription drugs, dental checkups, eye exams, glasses, and more.

Fidelis Care at Home
Fidelis Care at Home is a managed long term care program for individuals who need health and long term care services, who are eligible for Medicaid, and who require Nursing Home Level of Care. By providing long term care and other services to members in Orange and Rockland counties, Enrollment in Fidelis Care at Home is voluntary. Fidelis Care at Home allows members the independence of accessing long term care without having to leave their homes or communities.

Medicare is the nation’s largest health insurance program, covering 40 million Americans. It was designed to provide high-quality health care to people 65 and over, as well as to others with disabilities.

Fidelis Medicare Advantage
All the benefits of Medicare and more... for less. Fidelis Care is proud to offer Fidelis Medicare Advantage for Medicare-eligible residents of 22 counties across New York State, including the five boroughs of New York City, Long Island, and parts of the Hudson Valley, Capital Region, and Central New York. Key benefits include a $0 or low monthly premium (based on county of residence) and a point-of-service benefit with the freedom to choose your own doctor or hospital.

Albany Providers Join Fidelis Care Network

ALBANY, NY (June 30, 2009) – Fidelis Care is pleased to welcome two Albany providers to its expanding network: Senior Services of Albany, 25 Delaware Ave., Albany, and Senior Hope Counseling, Inc., 523 Western Ave., Albany.

Senior Hope Counseling, Inc., is a non-profit organization, licensed by New York State Office of Alcoholism and Substance Abuse Services, with the mission to provide high quality, science based, compassionate and comprehensive services to mature members of our community, 50 years and older, and their families.
